OSKALOOSA —
One individual involved in a two-vehicle accident 3:15 p.m. Sunday afternoon has died.
According to an Iowa State Patrol report, Lori L. Hazen, 45, of What Cheer, was driving a 1996 Pontiac east on Highway 92, two miles east of Oskaloosa. The report states that Hazen’s vehicle crossed the center line and hit a westbound 2000 Hyundai head on driven by Jessica L. Grahek, 19, of Oskaloosa.
The report goes on to state that both vehicles came to rest in the eastbound travel portion of 92.
Grahek and her passenger, Georgia Parashos, 18, of Norridge, Ill., were listed as injured in the Iowa State Patrol report.
The report states that Hazen was not wearing a seat belt at the time of the accident and that Grahek and Parashos were wearing seat belts at that time. All three individuals were transported to Mahaska Health Partnership, according to the report.
An investigation by the Iowa State Patrol is continuing and being assisted by the Mahaska County Sheriff’s Office, Oskaloosa Police Department, Mahaska County Ambulance and fire and rescue.
